Use the Student Organization/Club Attendance Sheet to keep track of all active members and the activities that they attend. Student Life will use your Attendance Sheet to validate membership entries on the Student Engagement Transcript.
In order to maintain active status, all Student Organizations are required to submit a Monthly Report form to Student Life and Student Government. This form encourages Student Organizations to reflect on accomplishments, goals, and learning outcomes, while allowing Student Life to monitor and track meeting activity, active membership, funds raised, community service activities, campus events, and/or conferences/workshops attended.
Use this template to guide you when giving your verbal monthly report at the Student Government Association General Assembly's.
Student Organizations must document their current officers, members, and advisors for the upcoming semester. Active Student Organization members should fill this form out confirming their anticipated participation in the Student Organization semesterly. The form must be completed and submitted for EACH and EVERY officer position.
Use this template to keep minutes following each meeting. Minutes should be posted in your Lion's Den Groups Page and sent to all officers and members within 2 days of each meeting.

Event Set Up Request Form
This form should be submitted after your budget request is approved; it is used to request audio/visual equipment, catering/food service, room layouts, and further logistics. Meet with the Assistant Director of Student Life if you have questions about the form.
All events hosted by Student Organizations are evaluated by Student Life to confirm learning outcome goals were achieved, and to calculate attendance at each event. After collecting the hard copies from attendees of the event, a designated Student Organization Officer should fill out this form with the information from each individual evaluation form.
This form is used when purchasing supplies from an outside vendor. Always submit this form any time a purchase is made for a Student Organization related expense.
Advisors and head officers are responsible for having all attendees complete this contract upon the student's acceptance to participate in off campus activities.
Promo Table Request Form
Use this form to request tables for bake sales, promotional events, etc. The requesting organization and its staff/faculty advisor are jointly responsible for the conduct of the organization named below. Your responsibility includes, but is not limited to, replacement or repair of missing or damaged college property. Advisors must be notified and pre-approve all events prior to submission of the Promo Table Request Form. Please confirm with Student Life within 24 hours of your request.
Photo/Talent Release Form
This form provides authorization to Raritan Valley Community College to utilize a students photo/likeness in any form, recordings made of voice or musical performances, and/or comments from any interviews, in whole or in part, for educational uses and in all forms of promotional materials. This usage could include but is not limited to: print, broadcast, video, film, social and online media and publications.

Student Organization/Club Budget Template
Student Organizations must submit their budgets semesterly. This template should be used as the template to keep track of all club/organization purchases, expenses, donations, etc.
Submit Conference Proposal and Budget Request Form at least 45 days before the conference. Student Organizations must make every effort to plan ahead and register during the "early bird" registration period. The budget must be approved before you can register. The purpose of the Co-Curricular Programs Fund is to provide co-curricular programs to assist in the development of educational, social, cultural, vocational, athletic, and leadership skills of all students. Students can request funding for a project/program by completing a Co-Curricular Event Proposal/Budget Request Form. Requests will be considered on an ongoing basis until the semester's deadline. Requests for the Fall semester will not be considered after October 1 and requests for the Spring semester will not be considered after March 1.

The Student Engagement Transcript is an official listing and validation of your leadership roles, community service, internship experiences, and much more. Official copies of the transcript may be used to complement your resume and academic transcript when applying to colleges/universities, prospective employers, or scholarship committees.

Submit the Request & Release Form to request an official signed and sealed copy of your Student Engagement Transcript. Only information that YOU have entered using the above entry form will be included on your transcript. Be sure to submit all experiences as they happen, you will have more difficulty verifying experiences a year later or several semesters later.
